I hereby request that you make all accounts associated with my Social Security Number or Tax ID Number available online. The statements for which I have online access will no longer be sent by postal mail. I understand that any additional statements I have requested be sent to others, i.e. accountant, tax preparer, attorney, or authorized signer, will no longer be available in paper form. Please continue to provide my statements electronically until I withdraw this request as described below.

You have the right to have the bank documents described above provided or made available to you on paper. You may withdraw your request to have electronic statements made available to you at any time by making the required changes in Business Online Banking. If you need assistance with this please contact us by e-mail at webmail@farmersebank.com or by telephone at 406-642-3431. If you change your e-mail address, you must make the necessary change within Internet Banking to continue receiving notifications of your electronic statement availability. You may request a duplicate paper copy of a document for a fee of $1.50 per page.

You understand that to receive and retain electronic disclosures you must have the following:

• Hard drive or printer to receive and/or retain Disclosures
• An electronic device with an operating system:
o Any operating system that will work with a supported browser
• Internet browser: Browsers and Operating Systems- Desktop

• Internet browser: Browsers and Operating Systems-Tablet

• Adobe Acrobat Reader (latest)
• Cookies enabled
• JavaScript enabled
• An e-mail account

Acknowledgement of Electronic Disclosures: I agree to accept Electronic Statements. I also agree to receive all future regulatory Disclosures including but not limited to Electronic Regulation E and Funds Availability disclosures and any change-in-terms notices ("Disclosures") in an electronic format. I hereby represent to you that I now have all the required hardware and software. You may change the requirements on 30 days advance notice via email. If I choose not to obtain the new hardware and/or software I may withdraw my request for Disclosures without any fee or charge by you for such withdrawal. I will remain liable for any other fees you have disclosed to me.
